Jack's Family Restaurants (JFR)

JFR will host a fundraiser as described above.
JFR will report the amount raised to Jack's Family Fund and beneficiary within 30 days of the fundraiser's conclusion.
Funds collected by JFR (or its representatives) will be transferred to Jack's Family Fund prior to disbursement.
Promotional materials and scripts being used by team members and/or volunteers to promote the fundraiser and ask for donations are available upon request.

Jack's Family Fund (JFF)

JFF will provide a dedicated online donation page with QR code upon request.
JFF will disburse fundraiser proceeds to beneficiaries within 30 days of receiving funds and final reporting from JFR.

Organization(s) receiving proceeds from this fundraiser will complete the disbursement form and submit a copy of their 501(c)3 determination (or other verification of their charitable status) online at jacksfamilyfund.org/fundraiser-disbursement
